Case Management

Utilization review and discharge planning begin on admission and continue throughout the patient's hospitalization.  An open referral system exists and is fostered at CRMC. 

All patients medical records are reviewed to determine intensity of service and severity of illness as mandated by CMS.  Patients with acute,chronic and/or debilitating illness or injury who have special post-discharge needs are offered appropriate services.
